Eligibility Prior to Tryouts
BEFORE THE ATHLETE MAY BEGIN PRACTICE/TRYOUT:
Athletes must be enrolled in Shorecrest full-time and all athletic forms in the Final Forms system must be completed. (Parent Form, Student Form, and Sports Physical Form).

Shorecrest must have an updated physical on file. Physicals last for two years. You may scan and email the form to Mr. Bruns (alan.bruns@ssd412.org) or ask your doctor to fax it to school 206-393-4284. You may also upload it directly to Final Form
Eligibility Prior to Competition
The athlete...
must complete 10 practices.
must pay for an ASB Membership for $50
must pay the District Athletic Participation Fee must for $200. This is for each season an athlete participates.
must have returned uniform / equipment from any prior sports season or paid a fine for any damaged or missing items.
